Stomach Ulcer in Nigeria: Early Symptoms, Foods to Avoid (Does Pepper Really Cause It?), and Safe Treatments

⭐ Clinical Truth by Pharm. Akpan Odudu-Abasi (B.Pharm, mPSN)

  • Pepper does NOT cause ulcers: Over 80% of peptic ulcers in Nigeria are caused by an infection with the bacterium Helicobacter pylori (H. pylori) or the chronic overuse of painkiller drugs (NSAIDs like Ibuprofen, Feldene, and Diclofenac).
  • Pepper may irritate an already raw ulcer crater, but eliminating pepper will never cure an underlying H. pylori infection.
  • Drinking cold milk provides momentary relief because it coats the stomach, but it stimulates acid rebound 30 minutes later, making symptoms worse.

Few health complaints in Nigeria are as ubiquitous or misunderstood as "stomach ulcer." From university campuses in Choba to busy executive offices in Port Harcourt and Lagos, thousands of adults endure gnawing, burning abdominal pain, bloating, and early fullness. Almost universally, friends and relatives advise them: "Stop eating pepper, drink cold milk, and buy antacid tablets."

As a community pharmacist, I frequently see patients who have restricted their diet for years, yet continue to suffer excruciating relapses. To permanently heal a stomach ulcer, you must understand what is actually happening to your digestive lining.

What Exactly Is a Peptic Ulcer?

A peptic ulcer is an open sore or crater that develops on the inner lining of your stomach (gastric ulcer) or the first portion of your small intestine (duodenal ulcer). Your stomach naturally produces potent hydrochloric acid (HCl) and enzymes to digest food. Under normal circumstances, a thick, slippery layer of mucus protects the stomach wall from digesting itself. When this mucosal defense breaks down or acid production overwhelms it, the acid burns the tissue, creating a painful ulcer crater.

The Two Real Culprits Behind Stomach Ulcers in Nigeria

  1. 1. Helicobacter pylori (H. pylori) Bacterial Infection: This spiral-shaped bacterium thrives in the acidic gastric environment. It burrows into the protective mucosal barrier, secreting toxins that trigger chronic gastritis and mucosal ulceration. In Nigeria, poor water sanitation and shared utensils facilitate widespread H. pylori transmission.
  2. 2. Overuse of Non-Steroidal Anti-Inflammatory Drugs (NSAIDs): This is the second leading cause. Taking Feldene (Piroxicam), Diclofenac, Ibuprofen, or Aspirin on an empty stomach for backache, joint pain, or menstrual cramps inhibits cyclooxygenase-1 (COX-1) enzymes, blocking prostaglandin synthesis which is vital for stomach wall protection.

Does Pepper Really Cause Ulcers? (The Myth Debunked)

Scientific studies—including clinical trials evaluating capsaicin (the active pungent component of hot chili peppers)—have proven that hot pepper does not cause ulcers. In fact, moderate capsaicin has been shown in laboratory studies to stimulate mucus secretion and increase gastric mucosal blood flow. However, if you already have an active, raw ulcer lesion, acidic or heavily spiced foods will chemically sting the open sore, provoking acute burning pain. Pepper is an irritant to an existing ulcer, not the cause.

Foods That Truly Aggravate Ulcers vs. Safe Healing Foods

Category Foods to Strictly Avoid / Minimize Recommended Healing Foods
Carbohydrates Fried plantain (dodo), puff-puff, highly refined white bread with margarine. Oatmeal, sweet potatoes, boiled plantain, brown rice.
Beverages Black coffee, energy drinks, carbonated sodas, alcohol, citrus juices (orange/grapefruit). Warm water, chamomile tea, cabbage juice (rich in glutamine), coconut water.
Proteins Deep-fried oily beef, heavily seasoned suya, fatty organ meats. Boiled eggs, steamed fresh fish, skinless chicken, plant-based proteins.

How Modern Pharmacists Treat Ulcers: The Triple / Quadruple Therapy

Antacid chewable tablets (like Gestid or Magnesium Trisilicate) only neutralize acid for 20–30 minutes. To truly eradicate an ulcer, clinical therapy requires:

  1. H. pylori Blood or Stool Antigen Test: Run an on-site H. pylori test at a certified lab like HandCare Diagnostics to confirm bacterial presence.
  2. Triple Therapy Eradication: If H. pylori is positive, your doctor or clinical pharmacist will prescribe a 14-day regimen combining a high-potency Proton Pump Inhibitor (PPI like Rabeprazole, Esomeprazole, or Omeprazole) plus two synergistic antibiotics (such as Clarithromycin and Amoxicillin).
  3. Mucosal Protectants: Sucralfate suspension coats and binds to the ulcer crater, shielding it from acid and allowing tissue regeneration.
